Hello
My name is Tonia and my daughter Valentina is 8yrs old
her ortho wants to do a surgery called a modified hoffer procedure. I am trying hard to find info about this. If anyone knows anything or could help I would appreciate it. Her pt seems to think we can get the range of motion from stretching and her ot does not seem to share that opinion. The only thing I know about the surgery is that it has to do with her shoulder and giving her more range of motion with it. They said their would be a cut in the front and one in the back of her shoulder. I don't know if there are other names for this surgery or not. This is what the doctor keeps calling it. I have posted a message before about this but I don't know what happened to it. We live in ohio and I would appreciate any advise or help. We just moved her last year and don't have any connection to any groups or others here. Thanks again for any help you might offer

Please, please slow down. I have never heard of the Hoffer procedure. This thought that there would be cuts to the front and back of the shoulder is so completely different from other very successful procedures that I have researched and studied. Please, please find a second opinion. Call the Shriner's Hospital in Philly fror a completely free consultation ....

no external rotation against gravity, can only go to 90 degree fwd flexion, no hand-to-mouth
1 surgery at age 14 (latissimus dorsi transfer). In 2004, at age 28 I was struck with Transverse Myelitis which paralyzed me from the chest down. I recovered movement to my right leg, but need a KAFO to walk on my left leg. I became an RN in 2008. - Location: Montreal, Qc, Canada

As far as I can find, it is a Latissimus Dorsi Tendon transfer used to increase abduction/external rotation.
If it is this, then I had this done at 14. I have a scar on the upper arm in back, and one on the front that runs down perpendicular to the armpit.
I had to do extenssive PT before (and after) to get more strength and as much ROM as possible. I was NOT able to get the Ext. Rot. without this surgery. I went from -180 to neutral (90) actively and more passively. (not sure if I am saying this right..it's been a while).

Katie had the latissimus dorsi tendon transfer on Sept. 16. Her cast comes off today. I have not heard it called the modified hoffer procedure, but it does sound an awful lot like what Katie had done. Her's was done by her ortho, Dr. Robert Goitz. Now as far as her ROM, I guess we'll start learning about that today. It was done for external rotation and abduction. I also understand that now we'll also be doing extensiver therapy. Katie was virtually unable to externally rotate at all. Anyway, if you find it is the same surgery, I'll be happy to keep you posted on our progress. (Well, even if it's not and you want to know, I'll be glad to share!!) I hope all goes well and you find the answers you're looking for. Take Care

The Hoffer procedure is a tendon transfer, etc. of the shoulder. Many people here refer to it as the "mod quad" which is the name the old Texas Children's Hospital bpi clinic called it when they expanded on it.
It is one of the original bpi surgeries.
Be sure to ask your ortho all of the questions you have--call back if need be. People here can give you links to websites, but you should be asking your doctor medical questions. Doctors often throw out phrases as if we have all just finished med school!!! You have every right to call back and ask for specifics on the surgery-what muscles he will be using, what he will be doing with them, etc.
My daughter had the modquad at 14 months old and gained tremendous range from it. She is now 6 years old.

hi my daughter had that operation at 2 years old our doctor called it tendon transfer it did work really well for her
