If you are talking about the medic alert cards? They can be found at Brachial Plexus Awareness page. Go there and click on Medical Professionals, then Physicians. Then there is an entry for Emergency Medical Card. It has choices to fit almost every mix of BPI.
Good Luck!!!

Injury Description, Date, extent, surgical intervention etc: LOBPI
no external rotation against gravity, can only go to 90 degree fwd flexion, no hand-to-mouth
1 surgery at age 14 (latissimus dorsi transfer). In 2004, at age 28 I was struck with Transverse Myelitis which paralyzed me from the chest down. I recovered movement to my right leg, but need a KAFO to walk on my left leg. I became an RN in 2008.

No, they used to do the bracelets for the kids either free or cheaper for us here on UBPN. I think they still do.. trying to find the posts...

What I am talking about, is a link to a pdf file that has various printings to explain to first responders (providing hey check your wallet)that you have a brachial plexus injury, which side it is on, ad certain precautions they need to exercise in moving, or positioning you, to be careful with injections to the bad side, etc. That may not be what angelsmom was talking about though.
Good Luck!!!

The bracelits seem like a better idea, especially for kids. In the absence of that, the alert cards are ok for someone who has a good place to keep them.
Good Luck!!!
