We started taking Matthew to a chiro in Feb. At that time he had a subluxation. We went weekly up until mod quad. At the time of surgery caps wasn't needed. Now after surgery things have improved and he only needs to go about once a month. She always checks him for dislocation before starting anything. I think regular chiro visits can help with this issue in some cases.

wow Cindy - reading your post gave me a dejavu in a way... just remembered how I could put Maia's arm 'back' and it would immediately fall right back out.... the first time Dr. Nath showed me this I got so nauseated. Then when I would massage her shoulder it would easily go back in and boom, fall right back out. Ugh. Guess you gotta have something to hold it in. I'm so grateful her caps surgery is holding for now and God willing for a long time!

That sounds so aweful, I'm glad Matthew's subluxation problem resolved so far from one adjustment. I can't even imagine knowing what that feels like. That is really great that her caps surgery is holding.
I've been meaning to email you for a while now, just been nuts dealing with the building of our house. Hope Maia's doing well.

Nettie, you may be interested in knowing that my daughter, Ashley, has never had any surgeries for her ROBPI but is likely to have the caps in her future. Currently her shoulder remains out of the socket by 1/4 inches but as yes does not cause her pain.

Can anyone share information or thoughts on the main idea of the mod quad being to balance the muscles to help with proper shoulder development? Many of us were told that range of motion was an important benefit but that the foundation of the surgery was for shoulder joint development. It seems that if the mod quad had done what it was originally intended to do then so many children who had it wouldn't go on to need the caps.

I am sure that not everyone who has the mod quad needs the caps but practically everyone I know who has had recent visits to TCH has been told their child needs it. Does anyone know if shoulder joint development is no longer one of the main objectives of mod quad? Also, I know several children whose injuries were not considered severe, nor did they need primary and are now being told they need caps.

It will be 2 years next month since my daughter had mod quad and when she was checked in september caps surgery was not recommended at this time. HOWEVER a triceps transfer is being discussed but for at a later time. Hope this helps answer your question.

Guest - I've sent your questions to Dr. Nath and hopefully (if he's in town) we can get him to answer this one. We're all pretty much surmising...best to get the answer directly I think.

My 14 month old son w/ right OBPI already has a dislocated shoulder. The ortho surgeon at Gillette in St. Paul MN told us she would do the caps and MQ at the same time. Anyone have experience with this? My son had primary at TCH in March. We are headed to TCH for a 2nd opinion on the MQ/caps later this month.

I asked the same type of ?s just over a year ago when we were deciding about having the Caps done on Nicole. What I learned then and since that time is that there are certainly children who need Caps but never had primary surgery. However, I think it would be very unlikely for someone to have the Caps done if he/she never had the Mod Quad (or similar type of surgery) done. I say that b/c I think the MQ is done to address issues that the Caps is also to help. So, I think if a child was a candidate for the Caps a MQ would likely help to some extent too (of course, this is just my non-professional understanding!). In some cases, the MQ would be done first and then the Caps would be decided upon later based on the MQ results. In other cases, I believe that both procedures might be done at the same time. I doubt though that the Caps would be done exclusively if the MQ hadn't ever been done. JMO. I'm tired so I hope this made sense. -Tina
