When my daughter was between the ages of 18 months and almost two, we traveled by air alot seeking medical opinions. I always contacted the Airline in advance and explained that I was travelling with a small child. Every airline provided a free seat for my child, provided that she sat in her car seat. They were always gracious and generous during the flight, and they always offered Airport transport service to make connections, etc...

I never sought services as a parent in distress, I just simply said that I was traveling for medical reasons with a faxed copy of our medical appointment and that I would appreciate any support. I just figured that the Airline had an interest in supporting the Hospitals that we were visiting...

I flew with Hannah to the camp in Texas when she was 2. She is now 7. She slept on my lap most of the flight, but once she woke up and the dude next to me had his tray down and a drink on it and she kicked her foot and his drink spilled,,whoops...also she loaded her diaper, but the flight attenedant was very helpful so I could change her pants. she laid a blanket on the floor next to their station and I also brought big zip lock bags for any messy diapers to dispose in.
We also flew for surgery to TCH a few months later.

Its basic common sense really, but following the tips really made our life a lot easier.
