obgyn
SD not mentioned to us but noted in record
8 lb 5 oz
2 weeks overdue, induced, epidural.
Vacuum suction
Baby in distress, in oxygen tent for 1st hour
We were told "temporary paralysis" and would be fine
in a couple weeks.

OBGYN delivered
SD noted in records - I honestly can't remember if he mentioned it in the delivery room
I again don't remember if any manuevers were used (awful, I know - but I was very out of it and in a lot of pain) - I believe that the record says something about using "all available methods"
Baby was 10 lbs. 10 oz. - induced at 38 weeks - previous babies were 3 lbs 9 ozs (33 weeks/induced/pre-eclampsia) and 9 lbs 2 ozs (38 weeks induced)
Baby was distressed at birth - spent 2 days on O2 (hood and CPAP) and a total of 8 days in the NICU

An OB delivered our baby
SD diagnosed
Possibly McRoberts used (I can't remember)
Suprapubic pressure used (I can only remember the nurse getting onto the bed, my husband said the Dr. yelled at her "NO like CPR!" I had the bruises on my pelvis to prove it too)
7 lb 8 oz
Dr. told us about the shoulder distocia in the delivery room, (and again the next day) told us about the bruises on our baby, and that they don't do anything for this injury until 3 months, if the arm isn't back to normal we will go to a specialist then. We believed her hook line and sinker.

Re: What type of doc delivered your child?
Ob's delivered both of my kids.
Melanie was 8lbs 12oz full term with no shoulder dystocia but bpi.
Matthew was 10lbs 6oz at 38 weeks with severe shoulder dystocia resulting in severe bpi. He used the McRoberts, lots of pulling, then finally used his fingers to sweep the shoulder free. Matthew's phrenic nerve was also damaged, required oxygen for 3 weeks, was intubated for 24 hours after birth.
