I would call the different doctors themselves and ask what they would suggest for internal rotation. I know the Mod Quad, including a pectoral release helped my child, but the injury is so severe and there are so many muscle imbalances that the internal rotation is coming back. It is time for us to make more videos, so I cannot help you as far as any advice we have been given recently. I would really just call the doctors and pick their brains. Every doctor I have spoken with (except one, and I would assume that you are not calling this particular specialist by the way your question was posed) has been exceedingly kind and helpful with all of our questions. If you do not like what the first specialist says, then move on down the list until you hear something that sounds good to you. Good luck with your search for information. When you find something out, would you mind posting it?

I would also love to hear about this I have been trying to find this answer out also. So far I am thinking that a Humeral osteonomy is one procedure that some are doing. This was recommended 18 months ago for my daughter we are now looking into the Triangle Tilt.

A few doctors do an arthroscopic capsule and subscapularis release to correct internal rotation contracture. Our son had this procedure at 13 months old. I'd be happy to answer via email any questions regarding our experience.

My son had the humeral derotaional osteotmy done for his internal rotation and we are very pleased. If you have any question I would be glad to answer them via email. I would be glad to share my experience with this surgery.

My son had severe internal rotation. Actually we were told by a well known doctor that he had "the worst internal rotation he had ever seen". I am not sure about that, but just wanted to express that he indeed had some serious internal rotation and was also dislocating. After much research, several opinions and MANY sleepless nights, we changed surgical plans and surgeons, and ended up having the anterior capsule release done to address the internal rotation. It is not easy and whose to say what the right choice is (only time will tell), but with certainty I can share, it is well worth your time and effort to research all possible options thoroughly, get multiple opinions and question, question, question.
Best wishes to you,

When did Tanner have the ACR? How is he doing now? How long was the splinting? Night time splinting? Aaron is on the waiting list for this procedure.

Hi Paula,

Yes, Tanner had the ACR. He is doing great. We are due for a 3 year follow-up MRI, so hopefully that will come back good, as it has in previous years that showed the shoulder joint actually remodeling....pretty exciting. In my son's case, he was very internally rotated. He gained much from the surgery....range, external rotation, a lot of supination, not dislocating, now able to get hand behind back (which even the doc told me he thought would never happen), elbow pretty much down instead of flaring out with hand to mouth or when walk/run, etc. Probably about 6-9 months post surgery, we saw a little internal rotation return, but it was very small in comparison to where we had come from and his elbow crept up a little, but since then it has stayed about the same, and it has been 3 years now, so I am just hoping things will continue to hold. As far as the splint...well, first he was in a pretty awful cast for about 4 weeks and then a splint for about 5 weeks or so, but that was mostly at nighttime only. I think it has changed since my son's surgery and perhaps it is different for different children based on their needs...not sure what the latest is.

Hope the surgery goes well for Aaron! When will he be having it and where are you having it done? Keep us posted.

Take care,
Lisa

We are on the waiting list for Houston Shriners with Dr Gogola. So they said it will be this summer for sure.

Paula,
Wow...I had no idea that particular doctor was doing the ACR. If you don't mind sharing, I would appreiate it...as it is always nice to have information to pass along to others.

Is this doctor a bpi specialist? Is that the majority of his/her practice?

How long has this doctor been seeing bpi kids?

How long has he or she been doing the ACR?

Do you know where he or she was trained on how to do this particular procedure?

Do you know patients that have been to he/she, and are they pleased with visits, surgery results and bedside manners?

Does this doctor work closely or collaborate with other bpi specialists across the nation?

Okay...sorry for the zillion questions. I am guessing others are interested as well and it would be helpful information.

If there is anything you don't feel comfortable posting on the boards, feel free to email me personally.

Thank you very much.
