Birth Records
New to the forum, i'm 22 and bpi since birth. I'm from Florida. I know it's different for every state but i'm wondering what the process is for finding your birth records. Do I contact the hospital? The only thing I can find online with the state is something about getting a copy of my birth certificate. I need to figure out the hospital I was born in first, for some reason I only have this dinky birth registration card and no certificate.

- Injury Description, Date, extent, surgical intervention etc: I am Mom to Amber, injured at birth. I serve on the Board of Directors for UBPN, and am a labor/delivery nurse, too.

You should contact the hospital where you were born, mostl likely the medical records department. You will have to send them a request in writing for a copy of your medical records. They may charge a fee for copies. Good luck.

I just went through this. Call the hospital you were born at. You want the Medical Records department. As you are only 22 they should still have them. They will probably require you to send them somehting in writing and charge you a fee but they will give you the address and such when you call. Good Luck.
